When IVF Works: Things the RE Doesn't Tell You

Okay IVF girls, here's the scoop as I learned it today from the doc himself.  Forget the googling, forget the note-taking, forget the comparing.   Just read...you'll feel better.  Promise.

Many of the following tidbits may apply to "regular" pregnancies as well, but the focus is for IVFers.

1) symptoms waning/fading:

Contrary to several sources on the internet citing that a pregnant woman who suddenly loses symptoms during the first trimester should be very concerned about an impending miscarriage, none of us in fact need be concerned.   At the onset of pregnancy when the initial hormone levels begin rising, the body reacts a certain way.  As it settles down, feeling better is just that- your body adjusting appropriately and becoming used to the hormones.   Throughout a normal pregnancy symptoms will come and go without warning.   A healthy body will strive toward homeostasis and it's best that your body feel good - you'll eat and rest more and the fetus reaps the benefits.  Symptoms may be reassuring, but they are overrated and pretty much serve just to make a pregnant woman miserable.  Your symptoms can be non-existent while your beta is tripling.  There is no correlation between the hcg and symptoms.  Having symptoms does not make one woman "more pregnant" than another.

2) IVFer symptoms waning:  At the onset of your pregnancy, your body was still coping with supernatural amounts of follistim, lupron, fading hcg, rising hcg and copious amounts of progesterone.   Many IVF women become sick upon a positive beta, especially if they have some mild hyperstim going on along with it.  I was bedridden for 4 days because my body's chemicals were raging.  These weren't symptoms of pregnancy; these were symptoms of my body coping with an incredible hormone surge.  My hyperstim was too mild to cause the sickness...that was all about ODing on hormones.  This, I found out today, is very COMMON.  Many women will overreact to the hormones initially, feeling very pregnant at the onset - bloated, nauseous, crampy, etc.   When that goes away you're just feeling better.  That's it.  It doesn't mean you're no longer as pregnant as you were in the beginning.   

3) but this isn't like my last pregnancy:

No two are the same, stop comparing, it's fruitless.

4) the hubbub about hcg:

HCG is only important at the first and second beta.  The first level should be high enough to suggest viability (my clinic uses over 50) and the second should demonstrate a good increase.  There is no magic number which ensures viability and conversely, a 51 should not cause alarm.   After that, beta numbers mean little, if anything.   Because the range of "normal" is so huge and individual pregnancies create hcg at varying rates throughout the first trimester, they can't (and aren't) relied on for anything but additional data to help assess situations.   They serve as a benchmark.  Interesting:  a dropping beta does not always mean a miscarriage.   My RE says he's seen all sorts of crazy hcg levels come in and lots of successful pregnancies come out of levels that other, less experienced reproductive people write off as doomed. 

4) Ultrasound benchmarks are just benchmarks.  Mother Nature will do things in her own way, in her own good time.   Yes, there are limitations and at some point you have to make a determination as to the viability of a pregnancy.   But just because someone sees a heartbeat at 5w5d and you don't, that doesn't mean your embie won't show the week after with a solid heartbeat and great measurements.   It doesn't mean your embie growth is lagging or "behind."  It means it's doing things in its own time and can be just as viable and healthy as the super-embie whose teeth came in at 7w.   And, oh yeah - hard as it is, as freaked out as you may be, don't encourage an early ultrasound unless something is really wrong.  You'll just do what I did today: push the issue for clarity and instead raise more questions.

5) Spotting:

IVFers SPOT.  You should probably expect to spot.  A normal IVF pregnancy will spot.   This is because the endometrium is so plump and cushy now and then bits of blood and tissue will squeeze out.  It's actually, ironically, a good sign - means your endo is one darn happy place for baby.  Of course spotting is not always good and should be monitored.  But normal pregnancies secret all sorts of colors of fluids at given times and that doesn't mean it's time to call the fat lady from her dressing room.  Generally, pink/brown/beige, watery or mucousy - it's all okay, in moderation of course. 

There may be more, but my brain is fried.  I'll update as I remember.  If anyone else has any IVF pregnancy myth-busters to share, please post.

I hope this can help some of you out there.

*Update June 2008* - this post was originally written in 2004. I have since moved on to have three beautiful boys (there's hope!) and currently write (hit "main" above if you're interested) about my family and crafting and such. If there is an interest (and apparently there may be, as I sometimes get thousands of hits to this post weekly), I will make a new page to allow for more info and sharing of IVF and 2 week wait information. If you are visiting here, please let me know if such a site would be something you'd want to see. I used to moderate an infertility support board, but do believe a site that serves "all your googling needs during the two-week wait" may be something useful, even though there are other types of such sites out there. Anyway, your input is appreciated. That said, good luck to all of you...IVF is an unbelievably trying process. You're not alone!

Thank you, thank you, thank you for the info about the symptoms thing. I was beginning to wonder if I was a total freak of nature.
My doc and I think also that the spotting I had was from the progesterone suppositories irritating me. My spotting stopped after I stopped the suppositories (I also felt way less irritated down there also).

I'm always amazed by the things doctors don't think are worth mentioning... Mine told me to expect bleeding like a period for 4-5 days post D&C. Instead, I got nothing at all for 5 days, followed by one large and scary gush. This sent me rushing to his office, followed by 2-3 weeks of a light trickle. All of which, apparently, was normal. So why not say in the beginning there are a range of possibilities and only X, Y, and Z should alarm you?

I was wondering if this information would apply to IUIers also ? I am 7 weeks PG. This is my second PG (had miscarriage 2 months ago). This is the furthest along I have gotten. I have been spotting (pinkish/brownish/beige and mostly only when I wipe) for past week. Has U.S. at 6 weeks. Showed heartbeat of 116 BPM but I am still scared about the spotting. I am hoping that it is only the progesterone suppositories. Have spoken with DR office, they say all is well and not to worry. Easy for them to say !!! Not going back for another U.S. until Feb 7th. It's gonna be a long 9 days !!! Anyways, thanx for letting me vent.

I honestly don't know of any women who DIDN'T spot after fertility treatments, especially those on progesterone. I'm so sorry to hear about your mc, but congrats on this pregnancy...it looks like this one is it for you! I know about how horrible it is to wait for that next u/s...believe me. Passing that heartbeat u/s is a huge hurdle though. Keep me posted!!! Hugs!

Hi There:

Things are really great right now !!! Had 8 week U.S. yesterday. Showed heatbeat of 173 BPM !!! Progesterone levels also good, but have gone down (56.9 at 6 weeks to 42 yesterday). Again, my DR office telling me no cause for concern, that fluctuating Progesterone levels are common (I am also on Progesterone suppositories which they tell me does not show up in the bloodwork because it is absorbed directly into your uterus). I guess I am a worrier by nature. Trying to remain optimistic and hoping for the best. Besides being really tired and some minor "tenderness" in breasts, I honestly do not feel pregnant. No "morning sickness" or anything like that. It's really weird !!! Oh, one last tidbit of good news. I "graduated" from my fertility clinic yesterday !!! Being transfered to my regular OB/GYN. My first appointment is Feb 25 (more waiting).
Well, I guess that's all for now. Hope everyone out there hangs in there and keeps their chin up. Amazing things can happen when you do....

I'm not sure where to begin. This is my 2nd invitro. my first was a fresh transfer of 3 which all 3 took initally. I started spotting, and hcg level dipped from 1600 to 700. I was told YOU HAVE LOST ALL YOUR BABIES there is no viable pregnancy. Needless to say, I had and still have a beautiful baby girl that just turned 2.

This is my second attempt. I had two Blastacyst ( frozen ) transfers 4/22 and confirmed pg on 5/5. My hcg level was 3008 and I was told " congratulations"... 2 days later I took another hcg test and it showed only going up 1/2 point . I was told by the same Jerk DR ( not my Dr.) that I had no viable pregnancy. I have been sobbing all day. In some way I don't beleive him, as he was dead wrong 3 years ago, but my heart is also telling me my baby is gone. My mom read something that said hcg levels don't always go up for 72-96 hours. Since mine went up 1/2 point, you'd think that I didn't lose my baby. I also think 3008 is the " Average" number even though it's a frozen embryo transfer. Who knows, to some degree I might still be wishful hoping. I'v been crying my eyes out and have a horrible knot in my tummy. If anyone could shed some light and hope for me, I would appreciate it.

You have no idea how you saved me today. I woke up this morning to go pee...nothing unusual...had been doing it all night at 2-3 hour intervals. This time, in the daylight, I decided to inspect the toilet tissue. 2 miscarriages after 2 IVFs in the past year tend to make a gal do these things as you know. There it was. The dreaded liquidy brownish discharge. I had seen it before with my first miscarriage and I immediately began to panic. I called my husband into the bathroom for a double inspect and in his usual manner, just told me to relax and that everything is fine. Easy for him to say. I wasn't going to get anywhere listening to his eternal optimism so I immediately went back to bed and started googling "brown discharge" and "early pregnancy" from my blackberry. Oh, that helped. In between googling sessions, I ran to the bathroom to pee and tissue inspect. More brown/pinkish discharge. I was losing it. Not wanting to call my RE for fear of facing what I perceived as the inevitable, I just kept peeing, tissue inspecting and googling...then I found you!

I'm only 6 weeks pregnant after my 3rd IVF (this time with PGD due to chromosomal abnormality with 2nd miscarriage...first one undetermined cause since I lost the pregnancy tissue at home). I'm also on lovenox for "slight clotting issue". 2 weeks ago I was also convinced I was miscarrying because I had severe lower back pains but turns out that was just OHSS. Gallons of gatorade later, the OHSS has subsided only to be replaced with the dreaded discharge this AM.

Anyhow, I read your entire blog and you have no idea how much your practical advice helped. I am done googling (at least for today) and will just return to A Beautiful Day if I start to freak again. (I can't promise that I'm done with tissue inspections though :)

Congrats on making it to the other side. If you have any other advice for this IVF/pregnancy induced neurosis, please share.

I am on the two week wait of my first IVF cycle and have not spotted one bit. So now you guys have me worried. I barely bled at all after the retrival (lots of pain though), and almost no spotting after the transfer. I'm on the prog. shots... 7 days today.

So should I be worried.

I had my fet on 06/03/06...I tested positive 11 days after and went back for another test 2 days later...it showed that my #'s doubled, but I'm spotting every other day...it's very light pink...or sometimes brown, I do not need to wear a feminine napkin or anything it's only when I wipe...it this normal?

I had my 2nd invitro treament about a month ago. Everythibg went ok, i had a transfer of 2 on the 29th of May. I started bleeding 2 days ago for a couple of hours, went to do the eco and showed everything was ok, just a bit of placenta moved. Yesterday i had my secong eco and no heart beat, but hormones are going up, so I am pregnant and very confused.??? Anybody had a late heartbeat?? Is it normal??...When should i worry??

Well, I had cramping yesterday afternoon through the evening and by the time I got home I had some bleeding when I wiped. I was so disappointed. This is my third cycle. I am on progestrone suppositiories, lovenox and estrace. Tomorrow is D-Day....I am scheduled for my pregnancy test. This morning I had some light spotting again. I called the nurses at the IVF clinic and they told me it could be from the progestrone. They said not to worry and get crazy and wait till tomorrow for the results. Today is going to be a long day!!!

is it normal that after the embryo-transfer I had todaz,to hav a brownish doschargewhen I wipe?! I am so worried! this is my 1st ivf/isci (blocked tubes and tried once iui before doing the hsg which was very painful as the fallopian tubes are closed). so, plsssss plssssss plsssssssss inform in case you have similar post ET reactions.

Thank you more than you know. I just got back a 54 beta and have been feeling crampy and yesterday had a brown spot and today some pink. You have made me feel so much better! I can not thank you enough!!!!

ivf/icsi transfer 10 days ago, starting spotting brownish (discharge). Very worried.

ON 2/12 I WENT IN FOR U/S, AND THE DR COULD NOT SEE THE BABY, I WENT AND GOT MY HCG LEVELS CHECK WICH WHERE 6080, I WENT BACK 2/14 AND MY HCG LEVELS HAD DROPED TO 5506, THEN TODAY (2/16) MY HCG WAS 6390. WHAT IS GOING ON???!!!??? PLEASE SOMEONE HELP ME, AM I PREGNANT OR NOT!!!!!!

Hi, thank you for addressing the issue of spotting. I had 3 rounds of IVF with the 3rd being the charm!! I am 10 weeks pg and I have been spotting for 6 days now. I went back to my doctor and they did the u/s and said everything looks good. This is my first baby at 36 and I just want everything to be perfect. I asked why would I have spotting if everything is good, and all they would say is that some women have spotting. One u/s technician did tell me that sometimes the vaginal suppositories may cause bleeding. Please update the site, this is the best info I have received!!!!! No more googling for me today!!!!

i have had two ivfs and turned out to be chemical preg. i switched drs and i will be going in on monday for pt test .i am on a new protocal this time .im taking prometreum and crinone gel and baby asprin. when i did my sweep today it looked black to me but i havent had any bleeding in panties are when i pee .has anyone had this do you lknow if norm and i feel pms crampy.?????

Help! I have had 7 IUIs 1 that resulted in a PG that miscarried at 9 weeks. I just completed my first IVF cycle beta test today only 36.2. Told by RN needs to be more than 100. Told to assume pregnancy with caution. I have a repeat beta scheduled in 3 days. I had no spotting after the procedure, no symptoms of pregnancy except mild cramping here and there. Anyone with any words of knowledge?

I just had a FET on 08/29/07 (1st cycle failed). I started to see some brown spotting on the 6th day which is still going on (today is the 15th day meaning it has been 9 days). Does this mean it's over? I have HCG level test scheduled for 09/13. But the wait is driving me crazy. I don't feel pregnant and other than a little breast tenderness (could be due to delestrogen shots), there's no other symptom. Any one had similar experience?

I am having my 12th IVF ( 2 miscariages over the years) had some spotting today, brownish/ DR says may be a good sign of implantation... that would be great!!!! Still 1 week to go before pregnancy test... each day feels like a week in itself. I am also on prgesterone suppositories... hurry up an wait.

So amazing to realize all us gals are going through the same things..

I am on my first Ivf cycle, i had sweats last night with cramps and headache. Woke this morn to brown discharge after insert prog supp. I have had brown discharge all day. Not heavy but consistent. I have my first blood test tomorrow. ET 01/10. Very worried although reading above has calmed me slightly.
Will let you know outcome tomoz.

I am on my first IVF cycle and have had everything going well so far. Today I got a lot of dark brown discharge in the morning and am now getting a light red watery discharge on the tissue when I wipe. I am also needing to go to the toilet a lot more often. I have never been pregnant before and do not have much experience on what is normal for a normal pregnancy let alone for and IVF cycle. I am taking Crinone gel once per day before sleeping. I was due for my first blood test on Monday but after talking to my doctor today and mentioning some spotting she has re-scheduled me for the test tomorrow morning. I am not cramping and am feeling tired in the afternoon lately. I have read that the brown chunky discharge is normal with crinone suppositiories and that spotting or light watery bleeding is normal for IVF. Can anyone please confirm this from experience. I would be especially interested if you have had a similar experience to me with your IVF and the outcome.

I am going to take it as a sign that I found this site today. I just completed my 3rd IVF. I am 5 weeks pregnant. My first beta was 152 but yesterday my beta dropped to 111. The nurse said, "this is not optimal". She said maybe it was a vanishing twin. I had 3 blasts put back so who knows. I have been crying every since. I have another blood test tomorrow and I pray that it goes up. I do not have any spotting and she said my progesterone level is good. I do not know what to think but I will take the info. you have here on hcg levels dropping. It definitely has lifted my spirits today. Thank you so much!

I was glad to find this site as well. I am on my third IVF attempt. The last time we only had two transfered on D3. Just before the test I started brown spotting, got the usual tension headaches, etc...and it came up negative. This tme, they trasnfered four on D5. Yesterday I had the tension stuff again which sent my mind in a million different directions. I have a little brown spotting though not as much as last time. My test is tomorrow and I am trying to stay positive!

I just had my first + beta @ 169 after 7 years of IVF (6 cycles not including FET's). The piece I just read just answered alot of questions and eased alot of anxiety. Thanks for sharing.

Ladies, I just had my 3rd IVF cycle (one fresh and one frozen before) and I started to lose hope that this will happen for my husband and I. I had some pink trace on the tissue last night and I freaked. I thought, here' we go again! But reading your blog really helped me calm down. I am too, doing the progesteron suppositories, the estrace and the aspirin. I am starting to think that spotting is a good sign now. On friday I have my blood test, wish me good luck. Alina

So, I had my PT and it was positive! My hcg was 326 (day 9 after frozen cycle). ON day 11 the hcg was 530 and today, on day 13 the hcg was 1234. The spotting
turned into bleeding for half a day but the doctor said it happens, just to stop the baby aspirin. Now it stopped completely and my first u/s is on the 19th. I am very excited. I wish the best of luck to everyone.

I am so glad to read everyones comments. This is my 1st IVF cycle and am doing acupuncture along with the cycle. Unfortunately, I didn't start acupuncture until after the transfer. This morning I started to spot along with some cramping and I was already planning for my 2nd cycle because I was sure the spotting was a sign that something was wrong. Hearing everyones story has made me feel a lot better. My 1st preg test is on 12/13. I'll keep everyone posted. Good luck!

For the last few months your page was the most reassuring thing that i have come across on the net. After having one failed ivf atempt we decided to have a second try, to our delight it was sucessful, after a positive result i had some spotting and various types of discharge. On my 5 week scan all seemed well, but i was anxious so we booked a second scan for the following week. It was amazing to see that little person with a heartbeat inside me. As we were put at ease we decided to leave our next scan until this week (i was 9 weeks). This time there was no heartbeat and the foetus had stopped growing at 6weeks 4 days.
I was bookind in for a d&c yesterday. It is one of the hardest things i think that i have had to deal with. But i know that i have to carry on,and look to the future. My husband and i are going to have a holiday and when we come back we'll be ready to face it all again.I just want to thank every one who has input on this page. I think that it honest and reasurring and i would be lost without it.

Have just found this page having had a bit of a bleed today 11 days after embryo transfer. Blood test on Monday - so glad to read everyone's comments and experiences - I also was starting to think about second cycle. Whatever happens on Monday, this forum has helped to make the wait to Monday less stressful. Many thanks


Let me start by saying Nicola,I'm sorry to hear about your lost, especially around the holidays. Hopefully your next cycle will work out.

This is the second day I have spotting but it is so light I can only see it when I wipe. The cramps have stopped and Im starting to think its the estrace,baby aspirin and progestrone shots I'm taking.I'm still nervous because my periods have never been very heavy and its too early to feel any REAL pregnancy symptoms.This wait is torture!I called my nurse and she said its no need to be alarmed and give her a call if it gets heavier. 6 days until the beta test.I am still hopeful but prepared for the worse.

The spotting stopped on Saturday morning and the tenderness in my breast has returned. Preg test on Thursday. I am so confused I don't know what to think. One thing I do know is I will try again if the test not positive.

I had my transfer 12/2/07 and my blood test is this Friday the 14th. This is our 1st IVF attempt (3 embryo's 70% chance of a live birth, through egg donation) and I'm so nervous. Friday the 7th I left work early due to bad stomach pains and nausea. I also experienced light cramping. IVF nurse said it could have been implantation or a virus and not to worry nothing could happen to the embryo's at this stage. The weekend turned out not so great. I ended up with severe diarrhea and would like to know if any of you experienced this? I'm afraid if this was a bug of some sort it will lessen the chances of any of my embyro's making it. I also experienced light pinkish discharge this afternoon when I wiped. I am also doing the Estrogen patches, Progesterone Capsules and Baby Aspirin. I must admit this site has helped me feel a bit better with some of my symptoms. Please keep your fingers crossed...

I had my first transfer 11/28/07. I went back 12/7/07 for blood test, Positive,180. Had to go back 2 days later for 2nd blood test, woke up that morning with brown discharge in underwear. I freaked, scared to death that the blood test must have been wrong and that I really was not pregnant. When I got to the dr. office they said that as long as the blood was not bright red then it was normal, my blood test 347, definitely pregnant. I spotted again the next morning but nothing since. This was my first IVF at age 32.

I also had slight diarrhea last week but nothing too bad. Last night I was feeling some tinges in my side and I felt like I had really bad gas. I'm so happy to hear your success on your first attempt, Trammell. My HD and I have been praying everyday for a positive test.

pregnancy test came back 360 last Thursday! We're pregnant! My HD is soo excited. Second test today,I hoping that the beta test climbs. I'll keep you updated. Best of luck to everyone.

Trammell: How ironic, I had an FET on 11/28 also! Congratulations on your pregnancy! My initial HCG was 787 then 2700 then today down to 2300. I'm sad to see it drop but DR says it may be multiples at first and now only one is growing?!? Sometimes I feel like I know too much. I have my first u/s on the 28th and we will get a better idea of what is going on. Good Luck!

Hi this is my first ivf after 13 years of no baby, had my transfer on 07/12 going for blood test on 21/12, had some brown spotting yesterday when going to loo but nothing today, lower back pain is this normal or a sign that ivf did not work.

I am 1 week post 5D transfer and just started spotting. Light, only when I wipe, either brown or watery pink. Also feel slights cramps like period is coming. Called doctor freaking out, and he said it is too early for period and that it is most likely irritation from progesterone suppositories or some implantation bleeding and not to worry. He also put me on oral progesterone in case I was not absorbing the suppositories. I am getting my test in 6 days.

I am 4 days post embryo transfer. I am so nervous. I feel like it will not work for me. I've had no spotting. I am nauseous (but on erythromicin). Does anyone have any words of wisdom on how to keep positive during this time? What is your protocol on bed rest after ET?

I just went through my first IVF cycle. We transfered 3 embryos on day 5 , but lost them.We have 1 embryo frozen.I scared , I'm 37 , no children.I'm afraid to lose this one too.Any suggestions ??

My 1st IVF failed. Conceived naturally after seven months but had early miscarriage during 6 weeks. Now I'm on my 2nd IVF, start to experience light brown spotting on 11th day of embyro transfer. Will have my pg test on Tuesday (day 13th). Can't concentrate with my work. I'm freaking out.

my daughter had an us last friday said the placenta measured 7 weeks, nothing about the fetus size, said no hb, went for hcg test went from 17,000 to 13,000 said to have a danc, no signs of miscarriage, any hope?

This site is so helpful. I have been freaking out all day. I am 8 days past 3 day transfer. I started experiencing spotting only when I wipe. It is pinkish and sometimes brown. I called my doctor's office today and was told it could be from the progesterone suppostories. They told me not to worry unless I start bleeding full flow. I have my first beta test on 2/26. I am trying to stay positive and hope for the best.

I am in my 2ww and started spotting 7 days post transfer, however the spotting is starting to get slightly heavier. Have I lostthis fight?

Hi all, your views are so comforting. I am 36, had first ivf, had embryo transfer (only 1 healthy egg) and 5 days later started spotting and then bleeding continually. I feel so freaked out, like a lot of you have been crying alot, I think aggravated by hormones.....have any of you had lots of bleeding and been cramping and still be pregnant. Have to wait 3 more days before testing.

I am on my 2nd IVF attempt and Day 9 post transfer of 2 Blastocysts. I'm very hopeful this will work for me. My blood test is 3/6. My husband thinks I'm crazy for spending endless hours googling. Little does he know that all of us obsess over the same things!!! I was so thrilled to find this site today. I too had light brownish/pinkish discharge and started to freak out. I'm trying to stay positive - 3 more days to go.

Hello, on Feb 25th I had my first IVF. Blood test on 3/1 showed that I am not pregnant! No words to describe my feelings! does anyone know when to expect my periods after my first IVF failure? Thank you so much!
